This certificate states that your pet is not a restricted breed or has any cross, mixed bloodlines containing such breed. This must be completed and signed by your veterinarian. Depending on your destination and / or airline each may have its standard form. If not, it is a good idea to obtain this from your vet on the veterinarian clinic letterhead, signed, and dated.
