Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
Antigua and Barbuda
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies. Pets must also be treated for parasites and meet all of the rabies requirements.
Aruba
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies. A health certificate must be issued within 2 weeks (14 days) of arrival into Aruba by a USDA Accredited Veterinarian. The animal has been inspected by an accredited veterinarian and has been found to be free of fresh or healing wounds, and clinical signs of screwworm. The animals originate from a zone which is not under control for screwworm, and have not transited a zone under control for screwworm within the last 60 days. As a veterinarian, I certify that the animal(s) described has been inspected by me on this date, within 2 (two) weeks of import into Aruba, and appear to be free of any infectious or contagious diseases and, to the best of my knowledge, exposure thereto, which would endanger the animal or other animals or would endanger public health. To my knowledge, the animal(s) described in box 21 originated from an area not quarantined for rabies and has/have not been exposed to rabies.
Bahamas
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies. You must fulfill rabies vaccination requirements according to the importation process and permitting.
Barbados
Leishmaniasis, Nipah virus, and Hendra virus are not endemic in export country. The animals have been tested within 30 days prior to export for the diseases with negative results: Heartworm (Dirofilaria immitis) – (not applicable to cats; or to dogs less than six (6) months of age)- ELISA antigen tests such as the IDEXX® SNAP® test kit may be used. The animals have been inspected by an accredited veterinarian and have been found to be free of fresh or healing wounds, and clinical signs of screwworm. 9. I, the undersigned Accredited Veterinarian, declare herewith that I have examined on the date indicated below, and within seven (7) days of travel, the animals described in box 21 and have found them to be clinically healthy, free from signs of contagious and infectious diseases, including rabies and external parasites and fit to travel. (For dogs only:) Within seven (7) days of export, the animals were treated for: a. Ticks with a formulation approved for that purpose; and, b. Tapeworms with an anthelmintic containing praziquantel. Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
British Virgin Islands
(a) The animal(s) are permanently identified with a microchip transponder or tattoo.
(b) The animal(s) are healthy and free from symptoms of infectious or contagious diseases.
(c) The animal(s) is /are treated against internal and external parasites.
(d) Dogs(s) has/have been vaccinated against Canine Parvovirus, Distemper, Hepatitis/Adenovirus, Parainfluenza, Leptospirosis and Lyme disease.
(e) Cat(s) has/have been vaccinated against Feline Calicivirus, Panleukopenia, Feline Rhinotracheitis, and Feline Leukemia.
(f) That the area of origin is not currently under Rabies quarantine.
(g) The animals have had at least two rabies inactivated vaccines; the first not earlier than 3 months of age and another at least six months later, and valid rabies certificate accompanies this certificate. (1yr and 3yr rabies vaccinations are accepted).
(h) At least one month after the second or subsequent booster rabies vaccines, a serological test (FAVN) vaccines, a serological test (FAVN) has been carried out to determine the presence of adequate virus neutralizing antibody titres (>0.5Iu/ml) prior to departure, and the laboratory results accompany this certificate. (The FAVN test results (>0.5Iu/ml) are considered valid for 12 months and need not be repeated if the animal re-enters the BVI within the same year.)

Cuba
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
Curaçao
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
- The animal must be vaccinated against Rabies at the age of at least 12 weeks with an inactivated vaccine at least 21 days but not longer than 12 months to 36 months (as registered by the manufacturer) prior to departure.
- The importation of animals younger than 15 weeks is not permitted
- The animal must be identified by means of an ISO (International Standards Organization)-microchip (mention the number and date of the application on the health certificate).
- The animal must be free of any ecto parasites. Therefore it should be treated with an systemic/oral or topical ectoparasite product as instructed by the manufacturer, that has a long lasting persistent efficacy of at least 28 days, 14 days prior to departure (mention the date and the product used on the health certificate).
- The animal must be free of any endo parasites. Therefore it should be treated within 14 days prior to the departure with a registered medicine as instructed by the manufacturer (mention the date and the product used on the health certificate).
- After treatments the animal must be kept free of any ecto and endo parasites.
Dominica
- The animal must be examined by a licensed and accredited Veterinary authority in the exporting country, and a certificate issued indicating the health status of the animal.
- The animal must be free from internal and external parasites.
- The animal must be tested and treated for Heart Worms and certified Heart Worm free for export.
- The health records indicating the Immunization Status of the animal must be provided with a current DHLPP+ C Immunization Record or any other endemic diseases of importance. Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
- Dogs and cats coming from all Countries must be immunized with an approved Rabies Vaccine by an official veterinarian stating type, serial number, date of expiration and date of administration, at least 30 days and not more than one(1) year prior to exportation of animal.
- A blood sample must be taken from the animal and sent to a reputable Rabies Reference Laboratory to perform the RNATT*/FAVN, and the result must be sent directly to the Veterinary Services prior to issuing an import permit.The test result must show a serum antibody level of at least 0.5 IU/mL.
Dominican Republic
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
The animal/s was/were vaccinated against rabies. The vaccination is current according to the vaccine period of validity, and in the case of first time vaccination, was given at least 30 days prior to departure. The veterinarian may certify the failure to implement the rabies vaccine because of insufficient age according to the vaccination schedule for the species.
This certificate is valid for 10 days after issuance.
Grenada
Your pet must receive a rabies vaccination within 12 months prior to arrival in Grenada. Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
Guadeloupe

Haiti

Tick and tapeworm treatment.
Jamaica
Helminths: Fecal examinations conducted and the dog/cat was treated twice by an approved veterinarian, at least fourteen (14) days apart with the final treatment seven (7) days prior to export. Treatment should be with broad spectrum anthelminthic(s) that is/are registered for use in the country of origin and effective against the nematodes, cestodes and trematodes. (State the active ingredients, product name, dose rate and dates of treatment).
External parasites: examination and treatment by an approved veterinarian TWICE using products registered in the exporting country for the control of flies, ticks, lice and mites at the manufacturer’s recommended dose. Dog/cat to be certified as free from external parasites at each treatment. The first treatment to be given 30 days prior to the date of shipment, at least two weeks before the second treatment, and the second within seven (7) days of export.(State the active ingredients, product name, dose rate and date of treatment.)
Leptospirosis: the dog was treated against Leptospira interrogans serovar canicola with an approved course of treatment within 30 days prior to the date of shipment; (State the active ingredient, product name, dose rate and date of treatment), OR The dog has been vaccinated against Leptospira interrogans serovar canicola and is up to date with booster vaccines.
Rabies: The dog/cat was vaccinated for rabies with a killed virus vaccine or recombinant vaccine expressing the rabies virus glycoprotein; Primary vaccination: the rabies vaccine was administered not less than six months and not more than one year prior to the date of shipment, when the animal was at least three months old. A copy of the vaccine certificate is required; OR Booster vaccination (older animal): the vaccine was given not more than one year prior to the date of shipment. A copy of the most recent vaccination certificate is required; AND A neutralizing antibody titration test for rabies was conducted at an official laboratory (World Animal Health Organization Authorized Laboratories for Rabies Antibody Test) in accordance with one of the methods described in the OIE Manual of Standards for Diagnostic Tests and Vaccines, and found to have at least 0.5 IU/ml of antibody in its serum.
Other Primary vaccinations – Each dog was vaccinated according to the vaccine manufacturer’s recommendations against diseases due to the canine distemper virus, canine parvovirus, canine infectious hepatitis virus or; canine adenovirus type 2 (canine respiratory disease complex) and canine parainfluenza virus not less than two (2) weeks prior to export.
Each cat shall be vaccinated according to the vaccine manufacturer’s recommendations against feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) not less than two (2) weeks prior to export.
Heartworm (Dirofilaria immitis) – If six months of age or older on the date of shipment, the dog has been subjected to a heartworm antigen ELISA test with a negative result in the 30 days prior to the date of shipment. ALL DOGS (regardless of age) have either been treated with a product registered for heartworm prevention at the manufacturer’s recommended dose in the seven days prior to the date of shipment or are up-to-date with a sustained-release injection registered for heartworm prevention.
Coccidiosis – Recognized tests. If positive, the dog must be treated with an effective product for the treatment of coccidial infections according to the manufacturer’s recommendations within 21 – 30 days of travel.
Leishmaniosis- Dogs must be tested for Leishmania infantum infection by an indirect fluorescent antibody test (IFAT) or an enzyme linked immunosorbent assay (ELISA). The test must produce a negative result within 45 days of export. This test may not be required if the country is free of Leishmaniosis. This should be stated in the certificate.
Babesiosis, Lyme disease and Ehrlichiosis – each dog has been tested using an approved test inclusive of ELISA for Babesia canis, Babesia gibsoni, Borrelia burgdorferi, and Ehrlichia chaffeensis with negative results within 16 days of export.
Brucella canis*: – The dog has been subjected to a rapid slide agglutination test (RSAT), or a cytoplasmic agar gel immunodiffusion test (CPAg-AGID), or a tube agglutination test (TAT) with negative results within 45 days prior to the date of export; AND The dog was not mated to a dog of lesser Brucella canis health status in the 45 days prior to the date of shipment. This test may not be required if the country is free of Brucella canis infection. However, this should be stated in the veterinary health certificate. *NOTE: this test is not required for dogs which have been spayed/neutered.
Canine transmissible venereal tumor – the external genitalia of each dog was examined no less than seven days prior to the date of shipment and no lesions suggestive of canine transmissible venereal tumor was observed.
Canine Influenza – The dog is free of infection with or signs and symptoms of canine influenza.
Each dog was examined within seven (7) days of export and found to be in good health, physically sound, free of any evidence of infectious disease and was fit for travel.
Martinique

A health certificate issued by a USDA Accredited Veterinarian and endorsed by USDA APHIS within 30 days of the date of arrival.
Parasite Treatment and Health Certificate
Your pet must be treated for internal and external parasites by a USDA Accredited Veterinarian within 72 hours of travel.
The accredited veterinarian will then need to issue a health certificate within 72 hours of arrival into St. Kitts and Nevis. This certificate does not need to be endorsed by USDA APHIS.
NOTE: Rabies titer results must be noted on the APHIS endorsed health certificate
Saint Lucia

Saint Martin / Sint Maarten
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
Pet must be 13 weeks or older at time of travel to St. Maarten.
The dog (s) has/have adequate levels of immunity conferred by being actively immunized against the following diseases: Distemper, Hepatitis (Adenovirus), Parvovirus, and Leptospirosis. The cat(s) has/have adequate levels of immunity conferred by being actively immunized against the following diseases: Feline Leukemia (FeLV), Feline Rhinotracheitis, Calcivirus, Panleukopenia and Pneumonitis.
Parasites:
a. The animal(s) is/are free of internal parasites as determined by a negative fecal flotation test (within 21 days prior to export) and were treated within 21 days prior to departure with a registered medicine as instructed by the manufacturer.
b. The animal(s) is /are free from external parasites, particularly Amblyomma sp. and Boophilus sp.
c. The animal(s) has/ have been treated for external parasites with a registered insecticide within 14 days prior to departure The Health Certificate must attest that It/they has/have been vaccinated against Rabies with an inactivated vaccine, and the vaccination is current. For animals receiving their first rabies vaccination, or a rabies booster after previous vaccination has expired by greater than 3 months, it/they has/have been vaccinated at least 21 days prior to shipment.
Saint Vincent and the Grenadines
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
The animal(s) were born in and has never left the approved country of export, OR where not born in or have left the approved country of export and returned, and have been in the United States for the previous six (6) months. 2. The animal(s) have been implanted with an I.S.O. microchip, prior to receiving a rabies vaccination with an inactivated adjuvant vaccine. The animal(s) were not less than twelve (12) weeks of age at the time of vaccination. 3. At least three (3) weeks following the rabies vaccination, a blood sample was taken from the animal(s) following microchip verification, and tested for antibody level via FAVN test at an approved laboratory*. (Once the animal has a protective antibody level (>0.5 IU/ml) as established by the approved laboratory, the animal can be exported to St. Vincent and the Grenadines. A copy of the Laboratory result must be delivered to the office BEFORE an import permit will be issued. No import permit will be issued until the results have been received at this office and the protective antibody level is confirmed.) 4. Upon inspection, the animal(s) were found to be in in good health and free from symptoms of any notifiable or any other infectious or contagious disease. 5. Dogs only: The animal(s) have been vaccinated against Canine Distemper, Parvovirus, Leptospirosis, Hepatitis and Parainfluenza not less than seven (7) days prior to date of export. 6. Cats only: The animal(s) have been vaccinated against Feline Rhinotracheitis, Panleukopenia, Feline Leukemia, Calicivirus and Chlamydophila psittaci 7. Dogs only: The animal(s) were tested for heartworm disease with negative results within seven (7) days prior to date of export. 8. The animal(s) were treated for external parasites and for internal parasites with an approved broad spectrum antihelminthic within seven (7) days prior to date of export.
Trinidad and Tobago
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.
- Pet must be vaccinated for rabies after implant of microchip.
- Pet must have a rabies titer blood test no less than one month after the rabies vaccination from an approved laboratory.
Turks and Caicos Islands
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines. The core feline vaccines are those for feline herpesvirus 1 (FHV1), feline calicivirus (FCV), feline panleukopenia virus (FPV), feline leukemia virus (FeLV – kittens) and rabies.

I further declare that the animal (s) listed on this certificate *has met /*have met the following requirements:
Permanently identified with a microchip transponder or tattoo. The microchip is able to be read by a microchip reader and that the chip was placed before the primary rabies vaccination was given;
The dog(s) *is/*are vaccinated against Canine Parvovirus, Distemper, Hepatitis/Adenovirus, Parainfluenza, Leptospirosis and *has/*have been vaccinated against Lyme disease in accordance with the manufacturer’s data sheet or *has/*have tested serologically negative for Lyme disease not more than 10 days prior to the date of shipment. (Idexx SNAP® 4Dx Plus® is acceptable for Lyme disease screening).
The cat(s) *is vaccinated/*are vaccinated against Feline Calicivirus, Panleukopaenia, Feline Rhinotracheitis and Feline Leukaemia according to the manufacturer’s data sheet.
That the animal(s) *was treated/*were treated against internal and external parasites within 14 days of the scheduled date of shipment: a. For ticks, a preparation known to have residual action should be used, e.g. products containing fipronil, amitraz, pyrethrins and permethrins. The following products are not acceptable: Shampoos, collars and products used for flea control only (e.g. Program®, Advantage®, Revolution® or Sentinel®)
For tapeworms (Echinococcus multilocularis), a product containing praziquantel given at a dose rate of 5mg/kg. Please note that the date of this treatment must precede the date the certificate is signed.
United States Virgin Islands
Vaccines for canine parvovirus, distemper, canine hepatitis and rabies are considered core vaccines.
